This on-line report is intended to only capture a summary of basic information. It should only be used to report accidents & injuries, minor property related security incidents and circumstances relating to potential College liability.

Security incidents that are not minor property related incidents should be reported by contacting the Security Office by phone (ext. 5010) or by visiting the College Security Office located at room A1216 at the Ottawa campus. Security Officers are available at all times to speak with you regarding your report.

Emergencies requiring immediate response should be reported by contacting the College 24 hour Emergency Line (ext. 5000) in accordance with the College Emergency Procedures.

Please note that the Occupational Health & Safety Act requires that a worker’s supervisor be advised of any accident that occurs in the workplace.
