Our Perth Campus Health Services Registered Nurse provides professional and confidential nursing services.
Registration for First-Time Visitors
If this is your first time using Health Services at Algonquin College’s Perth Campus, you must first register with our clinic by completing the Patient Registration and Consent form.
We will be happy to assist you in booking an appointment after you have completed and submitted form above.
Perth Campus Registered Nurse
Our Registered Nurse is at the Perth Campus on Wednesdays during the regular semester.

If able, please bring all of your personal immunization records with you – get copies from your family doctor, the Health Unit your high school was serviced by, or other clinics where you may have received vaccines.
Emergency medical services are available locally at the Perth Hospital Emergency Department: 33 Drummond St., Perth.
Start Early to get Medical Forms for Program Entry!
If you need to submit medical forms for your program, you need to plan ahead and start early. Forms must be submitted prior to field placements. The process usually takes a minimum of 2 weeks to complete, but may take up to 4 weeks depending on your immunization history. The nurse can help you with the completion of the forms and all required blood work, immunizations and TB testing.
PLEASE NOTE: Some programs have specific deadlines for form completion. If you have any questions regarding requirements or deadlines, please contact your program coordinator.
Health Services for Students
- Birth control sales, and emergency contraception – Plan B
- Emergency treatment for minor injury or illness
- Treatment for headaches, colds, etc.
- Flu shots (during flu season as established by Public Health)
- Health and nutrition counselling (stress, exercise, quit smoking, drugs, alcohol)

- Blood tests required for your programs medical form
- Tuberculosis (TB) testing
- Vaccinations, including Hepatitis B, tetanus, and MMR (mumps/measles/rubella)
In cooperation with Counselling Services and the Centre for Accessible Learning, we offer help with almost any problem you may have.
