Laurie Ann Klawitter is a Registered Nurse in Health Services and is on campus Monday to Friday from 8:30 a.m. to 4 p.m. to assist Algonquin College students. Laurie Ann is an experienced nurse who provides good advice on health care issues impacting your ability to be successful in your studies. She can also refer you to community health care resources where you can access additional support.
If you have any of the symptoms below Call 9-1-1 or go to the nearest Emergency Department:
- significant trouble breathing
- chest pain
- fainting
- significant worsening of any chronic disease symptoms
- severe injury

Flu Vaccination Clinic
It is more important than ever to reduce the spread of the flu.
Algonquin College’s Health Services has flu vaccines that are available for students and employees. To book an appointment, please call 613-735-4700 ext. 2748, or email, klawitl@algonquincollege.com
Please complete the Influenza Immunization Consent Form. Once complete, please bring the document to your appointment or email the document to Health Services at klawitl@algonquincollege.com before your arrival.
The Flu vaccine is also available through your health care provider or pharmacy. If you are having difficulty accessing the flu vaccine, please call Health Services.
In addition to getting your flu vaccine, you can help stop the spread of the flu, and protect yourself and your family, by following a few easy steps:
- Wash your hands frequently with soap and water or use hand sanitizer.
- Avoid touching your eyes, nose, and mouth. Germs spread this way.
- Cover your coughs and sneezes with your arm, not your hand.
- Maintain physical distancing by standing 2 metres (6 feet) apart.
- Wear a mask in all circumstances where required.
- If you are sick, STAY HOME.
- Do not visit hospitalized patients or residents of retirement homes or long-term care homes if you are experiencing flu-like symptoms.
- Abide by the regulations for maximum group sizes for indoor and outdoor gatherings.
Flu Vaccines will be available end of October.

Registration for First-Time Visitors
If this is your first time using Health Services at Algonquin College, Pembroke Campus, you must first register with our clinic by completing the Patient Registration and Consent form.
We will be happy to assist you in booking an appointment after you have completed and submitted the New Patient Registration and Consent form.

Booking an Appointment
To book or change an appointment with the Nurse at Health Services, please call us at 613-735-4000 ext.2748. Our phone lines are open Monday to Friday, from 8:30am to 4:00pm.
For every appointment you must bring both:
- Your valid provincial health card or GuardMe card or UHIP Card
Preparing for an Appointment
If this is your first time using Health Services, you must first register as a new patient. Please be aware that we cannot book an appointment for you until the completed forms below have been received by our clinic. Please note that Health Services is available only to students enrolled at Algonquin College and employees of the College.
Canadian Students
To register as a new patient, please complete the New Patient Registration and Consent Form and send this form back to us either by mail or email. You can sign this form digitally by following the instructions at the signature box. Unfortunately, we cannot accept forms in person at this time due to the COVID-19 situation.
International Students

Please note, there is an increased security risk that your health information may be intercepted or disclosed to third parties when sending your information by email, if not managed properly. Therefore, to help keep your information safe and secure, you should:
- Use your AC email address (****@algonquinlive.com) to send us your information. Using personal email accounts (e.g. Gmail, Hotmail, etc.) is highly discouraged for sending us your personal information.

Alcohol & Other Drug Support Services
Our Health and Counselling services offer a support network for those struggling with addiction or those living with someone who struggles with addiction. We also partner with the Renfrew County District Health Unit (RCDHU) that protects and promotes the health and well-being of all residents through leadership, partnership, accountability and service excellence. Healthy Attitudes
Algonquin College is committed to maintaining a campus free of discrimination and harassment. We strive to foster an atmosphere of healthy attitudes and behaviours towards sexuality, sex and gender roles. We are committed to supporting students who have experienced sexual assault/sexual violence, and we work diligently on campus and with community partners, to ensure that our campus remains free from sexual assault/sexual violence.
Consent is the active and willing participation in sexual activity for both parties involved. Both partners must have the freedom and capacity to make a choice. Consent means enthusiastic participation in sexual activity. Consent cannot be assumed. Checking for consent needs to be an ongoing process, and is the responsibility of both partners. An absence of a “no” doesn’t mean “yes”. If you’re not sure, it’s always best to ask. Remember that sex without consent is rape.

Immunizations for Placements
Review Your Immunizations
We strongly encourage you to review your immunizations with your family doctor, using our Immunization for Placements Sheet to ensure your health is protected while you pursue your studies at Algonquin College.
All required vaccinations and TB tests are available at the Ottawa, Perth and Pembroke campuses.
The process for immunizations can take 2-4 weeks or even longer in some cases, so plan ahead and start early!
Completing Your Forms
The Registered Nurse, Laurie Ann Klawitter, at the Pembroke Campus can help you with your Placement Pass Forms, free of charge. Please bring your forms, copies of immunization records, and your health card to Health Services.
Completing your Placement Pass Forms does not require the services of a physician. If your doctor fills them out for you, you may be charged a fee. If your Placement Pass Forms are completed by your physician, please ensure that he/she signs the document.
Entry Immunization Forms
An Entry Immunization Form is required for some programs. Up-to-date immunizations and screening for immunity to certain diseases will protect you against communicable diseases. The process can take 2-4 weeks, so plan ahead and start early!

Information About Immunizations
- TB testing is provided free of charge
- Hepatitis B vaccines are available at a reduced rate for Algonquin College students
- Free influenza vaccinations are offered during flu season
- Covid-19 vaccine is not yet available at our clinic
- If you require blood work, we can provide you with the necessary requisition form to bring to a lab. Blood work can also be done at the Ottawa campus.
- Some programs have specific deadlines for form completion. If you have any questions regarding requirements or deadlines, please see the placement requirements section on your program webpage.
Returning Students
Updated one step TB testing is required annually for health care programs.
If you’ve had a positive TB test, please contact Health Services to connect with a Registered Nurse to review and update your records.

Immunization FAQs
I was asked to complete my immunizations for my program placement, can I do all of the required immunizations at Health Services?
All required vaccinations and TB tests are available at the Ottawa, Perth and Pembroke campuses.
How long does it take to complete all of my immunizations?
Immunizations required for medical clearance for program placements or program admissions often take several appointments, depending on your medical/vaccination history. A timeline of 2-4 weeks might be required. Plan ahead and start early!
I have been asked to submit a medical form for immunizations for my clinical placement, where can I find the form and how do I complete it?
If you need to submit medical forms for your program, you can download them from your program website. Plan ahead and start early! Forms must be submitted prior to field placements.
Note: Completing your forms does not require the services of a physician. If your doctor fills them out for you, you may be charged a fee. If your form is completed by your physician, please ensure that he/she signs the document. The Registered Nurses at the Ottawa, Perth, and Pembroke Campuses can help you with your forms, free of charge. Call your campus clinic to book an appointment, bring copies of immunization records and your health card to Health Services.
Are the required immunizations for my program placement covered through insurance?
TB testing is provided free of charge. Hepatitis B vaccines are available at a reduced rate for Algonquin College students. Free influenza vaccinations are offered during the flu season. For all other immunizations, please check with our team.
