Additional Admission Requirements Submisson – Online Graduate Certificates

    This is a supplemental form to submit additional documents as per the eligibility requirements for the full-time graduate certificate program you applied to. Any submissions on this form without an active application submitted through OCAS, will not be processed. Please make sure that you apply on Ontario Colleges prior to using this form.

    Ensure that you have consulted the eligibility package requirements and have all the supporting documentation in a digital format ready to be uploaded. Required documents may include any of the following: Transcripts, Diplomas/Degrees, English proficiency results, Education evaluation for international credentials, Resumes, Reference letters, and other. You will have an opportunity to upload your documents on the second page of the application

    Once you have submitted the appropriate documentation, your application will be assessed and the registrars office will be informed of our decision. Additional documents submitted in a paper format or via email will not be accepted and incomplete applications will not be processed.
